REGISTRATION INSTRUCTIONS
How To Register
By mail:
Simply fill out the MCAA Conference Registration Form on the facing page. Indicate your choice of Advance Registration Fees and mail the completed form with your payment to:

Advance registration is strongly recommended to save time and money and to ensure availability of all the sessions. You may, however, register on site.
Registration Procedure
Fill out the name and address identification completely, indicating your telephone number and FAX number.
STEP 1-
Badge Information: Print the name as you prefer them on your badge for both you and your spouse/guest. Also indicate in the right hand column your class of membership.
STEP 2-
Advance Registration Fees: Select the package you desire and mark an X in the box provided and amount of fee in the right hand column. Please note that the Ladies Program is not included in the registration package. See Step 3 "Individual Function Fees."
STEP 3-
Individual Function Fees: Listed are the individual charges for many of the functions, with the MCAA Phoenix Open being one of those items. If you desire to play at the Arizona Biltmore Country Club on Thursday, February 1st, please indicate.
Total all charges from Steps 2 and 3. This is the total registration fee that should be submitted with this form. Please make checks payable to MCAA in U.S. Funds.
STEP 4-
Both the MCAA and the Masonry Expo Seminars will be handled on separate forms.
STEP 5-
Turn to pages 14 and 15 for your instructions to secure hotel rooms for the Conference and the Masonry Expo. MCAA will utilize the Sheraton Phoenix Hotel as their Headquarters.

MCAA will also sponsor "The Masonry Showcase" a live demonstration of Rough Terrain Forklifts, featuring the leading manufacturers in the United States and Canada of this type of material handling equipment. This demonstration will be Sunday morning, February 4, 1990, highlighting the capabilities of rough terrain forklifts exhibited on the EXPO floor.
MASONRY EXPO '90 will not only offer a shopping forum for the whole masonry industry but quality educational seminars and a chance to see some of the best and fastest masons in the industry during NCMA's "Fastest Trowel on the Block" competition. Six winners have surpassed local, state and regional competitions to participate in this event that will take place Monday, February 5, 1990 at 12:00 noon.
MASONRY EXPO '90-the masonry industry has waited a long time for such a valuable event, and three exciting days in February promise to live up to every expectation!

The MCAA Phoenix Open will be held at the Arizona Biltmore Country Club located in the heart of Phoenix. The 36 holes of championship golf hold a promise of challenge and beauty on every hole. At the clubhouse there is a fully stocked golf shop and a complete locker facility. Make sure you register for this big event.
